Applying for a Passport Name Change
Applying for a passport name change in the United States is a simple process but may require you to have ample time before the changes can be done. There are several requirements that you must satisfy, some of which calls for official documentation especially regarding your name change. Some of the circumstances that are allowed for a passport name change are in the events of name change through marriage, or through court order.
The requirements for applying for a passport name change in the US are as follows;
- If your passport was issued less than an year at the date of application for a passport name change, and that the name change was occasioned by a legal marriage or court order, then you must provide these documents; your current valid US passport, a completed form DS-5504, a certified copy or the original marriage certificate or the document issued by the government (federal or State law) as an evidence of name change and possibly a recent color photo. However, it is good to note that there are no fees associated with this type of name change unless you want to use the expedited passport services. It is advisable that you use secure mailing methods as well as confidential envelopes such as Tyvek envelopes.
- In the event that your passport was issued more than one year at the date of your application for the passport name change, and that the passport name change was occasioned by legally recognized marriage or court order, then there are requirements that you must supply. You must produce your current valid US passport and a completed DS-82 form. In addition to this, you must pay the applicable renewal fees and provide a recent passport photo. The documents ascertaining your passport name change must also be included; the original or certified copy of the marriage certificate or a government-issued official document supporting the court order on the name change.
- In the case that you cannot document legally the claim of your passport name change through marriage or court order, then there are other requirements that you must satisfy. You must present yourself at an acceptance center or passport agency with the following documents and conditions; your valid US passport, a completed DS-11 form, applicable application fees as the one for the persons applying for the first time, at least three public records that show your dates and place of birth and use of the supposed name for at least five years. You should also provide one recent color photo.
- When you are a minor and applying for a passport name change in the US, you should also present yourself at an acceptance facility or passport agency. The other requirements are as listed above as well as the special requirements that are supposed to be satisfied by minors applying for passports in the US.
